Continuing Care Coordinator/Discharge Planner

Talbott Recovery is currently looking for a fulltime Continuing Care Coordinator/Discharge Planner for our Main Campus on Hunter Road, Atlanta.  At Talbott Recovery we focus on patient-centered care and a personal individualized approach, upholding a high standard of innovative treatment and compassionate attention to each individual patient's needs. Qualifications

Position Summary

Responsible for the coordination of administrative and discharge planning for all Talbott Recovery patients

Essential Duties and Responsibilities

  • Establishes and maintains referral relationships.
  • Contacting referral sources upon admission and confirms patient's needs/expectations regarding updates/information.
  • Coordinates all communication and information exchange with referral.source.
  • Develops a comprehensive and patient-focused continuing care plan for assigned patients.
  • Timely and accurate input of contact information into established system.
  • Ability to access, assess and report on requested and/or needed information.

Education and Experience

  • Experience in alcohol/substance abuse/12-Step program.
  • Experience in Discharge Planning, Case Management or Continuing Care Management
  • Must have a High School Diploma or Equivelency; Bachelor Degree strongly preferred
