Job Description

Overview:

ORGANIZATION:  Milestone Centers Inc.

DEPARTMENT:  Service Coordination and ID&A Residential Program

POSITION:  Wellness Nurse Coordinator

STATUS:  Full Time; 37.5 hours / week

SCHEDULE: 9:00 AM to 5:00 PM; Monday through Friday

LOCATION:  Wilkinsburg & ID&A Residential Sites

Milestone Centers, a subsidiary of Partners For Quality Inc., is a nonprofit, community-based human services agency supporting individuals with behavioral health challenges and intellectual & developmental disabilities in communities throughout Western Pennsylvania. Milestone Centers has earned certification from the Sanctuary Institute for providing trauma-informed care.

Responsibilities:

Provides nursing care under the direction of Milestone’s Director of Nursing and providers. Serves as the Agency’s internal leader for the Behavioral Health Home Plus Expansion Program and the Intellectual, Developmental & Autism (ID&A) Residential/Life Sharing client wellness plan program.

  • Assists in educating the provider network about the needs of individuals with serious mental illness and complex medical needs with ID&A.
  • Analyzes individual and aggregate data
  • Participates in evaluation and data collection of clients served
  • Provides in collaboration with the Service Coordination Program and ID&A Residential/Life Sharing program, outreach to individuals who are identified on the registry or through Health Risk Screening Tool (HRST) and offer them individualized wellness coaching
  • Designs and conducts periodic health surveys for individuals with co-morbid conditions. Utilizes data provided to guide health related interventions to support the individual.
  • Provides case-specific training and consultation to Behavioral Health (BH) and ID&A staff who are working directly with identified clients.
  • Conducts training for staff and consumers on common health-related conditions.
  • When necessary, accompany individuals and / or Service Coordinators, House Supervisors to medical appointments to ensure they are receiving appropriate PH care and provide follow-up consultation and monitoring as needed.
  • Advocates for the health of the consumer by:
  • Referring and facilitating access to quality Physical Heath (PH) / BH services intended to meet individualized needs.
  • Facilitating access and coordination of PH / BH services.
  • Being responsive to the needs expressed by the individuals’ receiving services.
  • Coach individuals on how to advocate for themselves during appointments with health care providers.
  • Participate in meetings to ensure coordination of care. Communicate as needed with individuals, natural supports, and treatment team members as needed to ensure collaboration on individuals’ needs and provide health related education.
  • Participate in treatment team and disposition meetings at the request of the hospital.
  • Other duties as assigned.

Qualifications:
  • Licensed Practical Nurse (LPN) with 5 years of nursing experience preferably in Behavioral Health and/or Emergency Nursing and/or working with aging adults and individuals with intellectual disabilities and/or Autism.
  • Current Pennsylvania LPN License
  • Act 33, 34 and FBI clearances
  • Valid Driver’s License
Minimum Compensation: USD $30.00/Hr.
